How do I log in and complete a mission application?

Below is a video that will walk you through the steps to completing a mission application.

What happens once I complete the mission application?

You should receive an email from the mission office when you complete the application.
If a background check is needed, there will be a link in the email. Please complete it immediately.
Once your application and background check are completed you will be notified by the mission office within 2 weeks.

What happens once my application is approved?

The mission office will set up an account for each person.
A personal financial contribution of $150 for national trips and $300 for international trips in the form of a non-refundable deposit made to Lake Pointe Church must be contributed by the team member before fundraising can begin.

How do I get a United States Passport?

IMPORTANT

If you have NEVER had a U.S. Passport, you will apply in person at a Passport Acceptance Facility (post office, clerk of court, etc.). Check out this video for detailed instructions.

Step 1 Complete the Application

Visit the U.S. Department of State website. You will find all requirements for first time applicants, current fees and procedures. Complete DS-11 online and print, or you can print and complete using black ink only.

MOST IMPORTANT: DO NOT SIGN the passport application until instructed to do so by the acceptance agent.

Step 2 Provide Evidence of U.S. Citizenship

You must provide one of the following: Photocopies and notarized copies are not acceptable.

· U.S. Birth Certificate

· Consular Report of Birth Abroad or Certification of Birth

· Certification of Naturalization

· Certificate of Citizenship

Step 3 Photo

You must provide one photograph with your application. To avoid processing delays, be sure your photo meets all photo requirements. CVS, Walgreens, etc. offer passport photo services. We can also take a photo of you at the Rockwall Campus by appointment.

Step 4 Make an Appointment at an Acceptance Facility

You must submit the completed application and required documentation in person at a passport acceptance facility. Both parents must be present for children under age 16. Some acceptance facilities require appointments and others do not. We recommend calling ahead of time to verify their appointment requirements, hours they are open, and acceptable forms of payment.

Step 5 At Your Appointment

Remember both parents must be present for children under age 16. Prior to your appointment, make a front and back copy of the ID you will present to the acceptance agent. You will present one of the following IDs to the acceptance agent plus the front and back copy of the ID you will use.

· In-state, fully-valid driver’s license

· Fully-valid, undamaged U.S. passport (may be expired)

· Certificate of Naturalization or Citizenship (if the photo is still looks like you)

· Government employee ID (city, county, state or federal)

· U.S. military ID or military dependent ID

· Valid foreign passport

· Matricula Consular (Mexican Consular Identification, commonly used by the parent of a U.S. citizen child applicant)

 

You will be presented with 3 options: Passport BOOK, Passport CARD, or BOTH.

The Passport BOOK is required for all Lake Pointe International Mission Trips

Processing Times

Routine Service approximately 4-6 weeks for the application process.

Expedited Service approximately 2-3 weeks for the application process. ($60 additional fee)

How do I renew a United States Passport?

Use Form DS-82 to renew your passport and the following must apply:

·Can submit your most recent passport with your application.
·Your passport is undamaged (other than normal “wear and tear”)
·Was issued when you were age 16 or older
·Was issued within the last 15 years
·Was issued in your current name (if you have had a name change, you must provide a certified copy of the legal name change document)

IMPORTANT
If any of the above statements do not apply to you, you must apply using form DS-11.

Step 1 How to Apply
Complete form DS-82 online and print or print and fill out using black ink only. Visit the U.S. Department of State website.

Step 2 Supporting Documents That Are Needed
You must submit your most recent passport with your application.
Include name change documents (if applicable). Name change documents include:
·Marriage Certificate
·Divorce Decree
·Court-ordered name change document

Step 3 Photo
Attach one photograph to the application. CVS, Walgreens, etc. can make instant passport photos. We can also take your photo at the Rockwall Campus by appointment.

Step 4 Passport Fees
For a list of renewal fees or other passport fees, please see the U.S. Department of State website.
Make check or money orders payable to “U.S. Department of State”. Print the full name and date of birth of the applicant on the front of the check or money order.

Step 5 Mail
When mailing your passport application, please use:
·An envelope large enough to fit the application without folding
·A trackable delivery method
Mail your completed application, including your most recent passport, name change document (if applicable), photo, and fees, to one of the addresses below.

FOR ROUTINE SERVICE:
Mail the completed application to:

What immunizations do I need?

The Center for Disease Control (CDC) is the best source for this information.

Step 1

Go to wwwnc.cdc.gov/travel.

Step 2

Select the country you will visit and answer a few questions about “What kind of traveler are you?”

Step 3

Contact your local physician and schedule time to receive the appropriate immunizations, or, contact a service like Tom Thumb Pharmacy Travel Immunizations.
